Name of the Patient : Abc Xyzlmn / M / 72 yrs.
Referred by : Dr. Abc Xyztchha.
Examination : M.R.I. of the Lumbo-sacral Spine.

CLINICAL PROFILE :

C/O backache radiating to BLE since 15-20 days.

EXAMINATION :

M.R.I of the lumbo-sacral spine was performed using the following parameters :

5 mm thick T1 Weighted and T2 Weighted sagittal images.

5 mm thick T1 Weighted and T2 Weighted axial images.

OBSERVATION :

There is a postero-central disc herniation at the L3-L4 level with anterior compression of the thecal sac and canal stenosis. There is inferior migration of the extruded disc portion which is seen to impinge the right L4 nerve root. The portion of the disc is also seen to migrate superiorly.

Schmorls nodes are seen in the dorso-lumbar region. The L4-L5 and L5-S1 discs are reduced in their height and the lumbar intervertebral discs show loss of water content. Hyperintense areas in the L5-S1 disc on the T1 Weighted images would represent calcification/paramagnetic substances.

The lumbar vertebrae show diffuse fatty change suggestive of osteoporosis.

The facet joints and the visualized pre and paravertebral soft tissues are unremarkable.

The conus medullaris terminates at the D12 level and the thecal sac terminates at the S1 level.

The antero-posterior dimensions of the lumbar canal at the level of the intervertebral discs are as follows :

18.0 mm at L1-L2
17.0 mm at L2-L3
8.0 mm at L3-L4
12.0 mm at L4-L5
12.0 mm at L5-S1.

IMPRESSION :

A postero-central disc herniation at the L3-L4 level with canal stenosis and inferior migration of the extruded disc portion impinging the right L4 nerve root.
