Name of the Patient : Abc Xyz Almn / M / 22 yrs.
Referred by : Dr. Abc Xyzah.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

H/O fever at the age of 14 years.
C/O gait imbalance and speech disturbances since then.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.
5 mm thick FLAIR coronal images.

OBSERVATION :

There are subtle, ill-defined, hyperintense areas on the proton, T2 Weighted and FLAIR images in the periventricular white matter bilaterally. These lesions appear iso to hypointense to normal white matter on the T1 Weighted images.

There is mild dilatation of both the lateral, third and the fourth ventricles. There is a hypointense signal, more pronounced on the proton and T2 Weighted images in the posterior third ventricular region, aqueduct and the fourth ventricle which suggests increased CSF flow. There is slight prominence of the cerebral cortical sulci, cerebellar folia and the basal cisternal spaces bilaterally. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.

The inferior vermis appears hypoplastic.

IMPRESSION :

1. Mild communicating hydrocephalus.

2. Subtle altered signal in the periventricular white matter bilaterally may represent transependymal CSF seepage.

3. Mild cerebral and cerebellar atrophy.
