Name of the Patient : Abc XyzJamkhanalmn / M / 67 yrs.
Referred by : Dr. Abc Xyzni.
Examination : M.R.I. of the Left Knee Joint.

CLINICAL PROFILE :

C/O pain in left knee since 7 months with occasional clicking.

EXAMINATION :

M.R.I of the left knee joint was performed using the following parameters :

4 mm thick T1 Weighted, proton and GRASS sagittal images.

4 mm thick T1 Weighted and STIR coronal images.

4 mm thick GRASS axial images.

OBSERVATION :

Menisci

There is a heterogeneous, hyperintense signal on all the pulse sequences in the posterior horn of the lateral meniscus of the left knee joint and which most likely represents a complex tear.

A linear hyperintense signal on all pulse sequences in the posterior horn of the medial meniscus extending upto the inferior articular margin represents a (? horizontal flap tear).

There is reduction in the medial tibio-femoral joint space.

The anterior horns of the lateral and medial menisci reveal normal configuration and signal characteristics.

Cruciate Ligaments :

The anterior and posterior cruciate ligaments show normal contour and signal characteristics.

Collateral Ligaments and the Patellar Tendon :

The medial and lateral collateral ligaments and the patellar tendon are normal.

Hoffas Fat Pad :

The Hoffas fat pad shows streak of hypointensity on the T1 Weighted images within it.

Articular cartilage and bones :

There is reduction in the medial tibio-femoral joint space with thinning of the articular cartilage overlying the medial femoral condyle. Subchondral bony irregularity/eburnation is noted. Marginal osteophytes are noted around the left knee joint. Fluid is noted in the suprapatellar bursa and around the left knee joint with probable synovial thickening. Suspicious loose bodies/synovial hypertrophy is noted in the suprapatellar bursa and around the cruciate ligament which appear hypointense on the T2 Weighted images (scans 102.9, 10, 13, 104.9, 10, 14, 107.4, 5). Subchondral erosion is also noted along the medial tibial plateau.

IMPRESSION :

Osteoarthritic changes in the left knee joint with :

1. Complex tear of the posterior horn of the lateral meniscus.

2. A (horizontal flap) tear of the posterior horn of the medial meniscus.

3. Reduction in the medial tibio-femoral joint space with thinning of the articular cartilage overlying the medial femoral condyle and subchondral bony irregularity/eburnation.

4. Marginal osteophytes around the left knee joint.

5. Fluid in the suprapatellar bursa and around the left knee joint with probable synovial thickening and suspicious loose bodies/synovial hypertrophy in the suprapatellar bursa. Pigmented villonodular synovitis should be ruled out.
