Name of the Patient : Abc XyzHalmn / M / 70 yrs.
Referred by : Dr. Abc Xyzrel.
Examination : M.R.I. of the Brain.

CLINICAL PROFILE :

C/O headaches since 3 months.

EXAMINATION :

M.R.I of the brain was performed using the following parameters :

5 mm thick T1 Weighted, proton and T2 Weighted axial images.

5 mm thick FLAIR coronal images.

OBSERVATION :

There are hyperintense foci on the proton, T2 weighted and FLAIR images located in both periatrial regions and in the right frontal region adjacent to the Sylvian fissure.

Dilated perivascular spaces are seen in lentiform nuclei, corona radiata and centrum semiovale bilaterally.

Mild fullness of both the lateral ventricles is noted. The third and the fourth ventricles are normal. There is prominence of the cerebral cortical sulcal spaces. The basal cisternal spaces are prominent. There is no shift of the midline structures. No obvious vascular anomaly is identified on this study.

Inflammatory changes are noted in the ethmoidal and frontal sinuses.

IMPRESSION :

1. Altered signal intensity foci in both periatrial regions and in the right frontal region adjacent to the Sylvian fissure are most probably ischemic in etiology.

2. Age related cerebral atrophy.

3. Ethmoidal and frontal sinusitis.
