Referred by : Dr. Abc Xyzra.
Examination : M.R.I. of the Right Shoulder.

CLINICAL PROFILE :

C/O pain in the right shoulder since 2 months with difficulty in lifting the RUE since then.

EXAMINATION :

M.R.I of the right shoulder was performed using the following parameters :

4 mm thick T1 Weighted and T2 Weighted axial images.

4 mm thick T1 Weighted and T2 Weighted sagittal images.

4 mm thick T1 Weighted, Proton and T2 Weighted coronal images.

OBSERVATION :

There is seen a small, well-marginated hypointense lesion on the T1 Weighted images in the head of the right humerus supero-medially. This lesion remains predominantly hypointense on the T2 Weighted images with focal hyperintense areas within. There is loss of normal contour of the right femoral head. There is reduction in the right gleno-humeral joint space. The articular cartilage overlying the right humeral head appears irregular. Fairly large, osteophytes are noted along the medial and inferior margins of the right humeral head. An infero-medial osteophyte is seen to abut against the inferior glenoid labrum with a probable tear of the inferior glenoid labrum.

Fluid is noted in the right shoulder joint with probable synovial thickening around the gleno-humeral joint.

There is slight thinning at the musculo-tendinous junction of the supraspinatous muscle with slight decrease in the bulk of the supraspinatus muscle.

The acromio-clavicular joint is normal. The acromion process is seen to sloping posteriorly and laterally.

The visualized axilla is unremarkable.

Fluid is noted around the biceps tendon in the bicipital groove.

IMPRESSION :

1. Altered signal intensity in the head of the right humerus, supero-medially as described may represent an area of osteonecrosis.

2. Severe osteoarthritic changes in the right shoulder joint with osteophytosis along the medial and inferior margins of the humeral head, reduction in gleno-humeral joint space and irregularity of the articular cartilage overlying the humeral head. Probable inferior labral tear is noted.

3. Effusion with synovial thickening in the right shoulder joint.
